Opt out not working - local sugar instance behind firewall

I'm new to Sugar and need some help please.    When we send email campaigns, if the recipient clicks the option to "opt out" of the campaign they are presented with the index.php code that resides on our website. I was guessing this is a permissions issue on the index.php file that was copied to our webserver perhaps? Does it need execute permission?

 

Our sugar instance resides on our internal network and the link sent in the email campaign points to an index.php file on our external webserver.
