In conducting an audit, I need your guidelines on how to make it easy and successful? Sometimes mistakes are unavoidable. How do we avoid it? Experts around, any advice for a better audit management?

You can create some reports which will show errors in records or process flaw in records and schedule those reports on daily or weekly basis to the users. reports can be schedule in the morning so that users can get
1. Create a report say Contacts Without Contact Details and this report will show records where phone number and email address is empty.
2. Create a report Leads Not Followed Up - This report shows records either with new status or assigned status and its date created is say last 7 days,
or you can create a field in leads module for next followup date and create a report Missed Followup in this report set filter next followup date is yesterday which will result in all leads where next followup date is due.
This way few reports can be created to avoid mistakes.
I appreciate your ideas! I think this would be useful on our part because we only have a small-medium organization. Anyway, I have a colleague who is always insisting to use a software. Do we really need to hire a provider just to help us with this matter? Thank you!

If the feature can be found within your CRM, I think there's no need to hire an audit management system provider to do the work.
Mr. Patil is on the right track. This is what we have advised to our customers.
If you have a large number of errors in data you might export them into Excel, or use the Sugar Excel Plugin. This would allow you to quickly identify records in bulk and to make changes en-masse.
If the issues are things like 'records without email addresses' consider using a work flow to alert the record owner. Make the WF timed so that they have X days to fix the problem before you escalate the issue to a supervisor. You should also provide a mechanism for the record owner to specify why the data is missing or wrong.
