I'm trying to create and email relating to Cases. How do I include Account information (with my test variables 1 - 5)?

Some findings (On Sugar 7.7), using SierraCRM Process Manager -
(1 - account name) I added a custom relate field to Cases called account_name. Now using $acase_account_name_c in the email template show the account name.
(2 - case number) I get different results for $acase_case_number between an on-site and on-demand instance. On-demand, $acase_case_number ends up blank. A workaround was to create a custom calculated field on Cases called casenumber_c, with the formula toString($case_number). In the email template, $acase_casenumber_c now shows the case number.

Your screenshots look different to my Email Template Editor In version 22.214.171.124 (and 126.96.36.199). Where do yours come from?
Mine looks like this below. In any case, inserting the account name inserts $account_name which just resolves to an empty string in the generated email.
From which module are you sending this email template?
If you trying to do this for forgot password for CRM users then this account name will not going to use.
For this you need to add user name from users module.
For that you need to go to Password management & from "Email template" section you can edit this template as below :
