How to create a custom audit table

Hello community!

any idea how can I start on creating a custom audit table for my CRM.
I'm currently using CE 6.5, and trying to create a custom audit table for the Case Module with fields as follow:

Name (User)   |    Remarks    |   Date Change.

To further explain, once I've created a case and assigned it to user x, it's suppose to show on the audit table along with its remarks, and in an instance that user x passed it to user y, it'll create another row showing user y's name and remarks. Somehow related to CE 6.5's current audit log, but instead of Old Value and New Value.
