Showing Target history after conversion to Lead and Contact

We are using Targets and logging emails, Notes, Calls as they happen. Once a Target is converted to a Lead, none of this data is showing under the new Lead.  That was the whole point to keep this data throughout the process for tracking our email efforts.   Using Act-On for email campaigns and it is properly logging each email as a Note in Sugar and updating the Score field. How can I get the Note field to say with the person as they progress through the pipeline?
