Dependent fields vanishing upon save

Dependent fields which are enabled upon selection of a certain option in a drop-down list in the account creation screen are visible whilst creating a new account, however after saving this account vanish once again, only to reappear if the specified field is deselected and then subsequently re-selected.

The bottom five fields are enabled after selection of the checkbox in the top right hand corner, which is enabled through selection of the "Superannuation" industry. 

In the saved account, all of these fields vanish.
