Error - Hundreds phone calls being added

Hi, hoping someone can help with this error.

We are using Sugar CE, version 6.5.20, and when adding a telephone call from Account > Activities > Log Call, upon saving the call, the CRM appears to get stuck in a loop and the browser ultimately needs to be closed. Upon reopening and logging back into the site, when looking at the Calls section, there are hundreds of blank phone calls added to the system which need to be manually removed.

Any suggestions to resolve issue would be greatly appreciated.
