Failed to Copy error when trying to install custom module

Version 6.5.20 Community Edition, Windows Server, IIS 7.0, and Microsoft SQL database.  I am attempting to create a custom module.  The module was created and published.  I have also downloaded it and tried an install from those files, but it keeps coming up with the same error no matter what I do.  I have checked the rights of the IIS_USR and it has full rights to the sugar directory.  I have updated php.ini and increased upload_max_filesize = 20M and post_max_size = 20M.  I keep getting this error (more lines above, but this gives the idea):

Rebuilding Vardefs...
Rebuilding JSGroupings...
Installing Images
Failed to copy cache/upgrades/temp/837D.tmp/icons custom/themes

I have installed other pre-packaged modules and they install just fine.  It seems to only be the Custom Modules.

Does anyone have any ideas on how I could fix this?
