Second email field pops up in Contacts form when converting Lead.

To convert a Lead, we have the Email field for the Contacts record required for conversion. However, it is automatically creating a second email field (as if I had pressed the "plus sign" symbol to add another email). This does not allow the conversion to be saved and salespeople have to go back and click the minus symbol to delete the second email field to save and convert.

Never had a problem with this before, just started happening yesterday.

Any suggestions on how to solve? Using Sugar 7 Professional.
