Notify on Assignment email not working

Users are not receiving a notification email when leads are assigned to them(among other assignments).

SugarCRM Professional, Version (Build 1118)
SMTP email settings are set up and the test email works.
The check box in the user settings and under the email settings is checked.

Did we miss something? Is there any solution besides creating a workflow?
